The American television series Couples Therapy is broadcasted on the Showtime network and has been renewed for a second and third season. The second season premiered on April 18, 2021, while the extended third season premiered on May 13, 2022. The show has also been shown on BBC Two in the UK and can be watched for free on the BBC’s iPlayer service. The third season filmed new couples and finished filming towards the end of 2022. It premiered on April 28, 2024, with the entire back half of the season available for streaming and on-demand viewing on this date, with linear airings of two episodes per week.
The Couples Therapy television series aims to provide an authentic look at the therapeutic process by featuring sessions of three to four couples per season, instead of creating drama for the audience. The series features Dr. Orna Guralnik, a couples therapist, who meets her patients for the first time when they enter her practice. The therapy sessions take place in a room with cameras hidden behind a one-way mirror, and the couples participate in a 20-week program. The one-hour sessions are recorded, edited, and cut into nine episodes for the television series. The show is filmed in New York.
The therapy sessions in Couples Therapy are subject to certain rules. Couples are not allowed to discuss their children in-depth as the children have not given consent for their lives to be discussed on television. The couples-only interact with the therapist, Dr. Orna Guralnik, and have no contact with the crew members. Dr. Guralnik has a counselor, Virginia Goldner, with whom she discusses the therapy process, while in the Covid Special, she works with Dr. Kirkland Vaughans. In June 2021, it was announced that the series would be renewed for a third season, which premiered on May 13, 2022.

Annie and Mau found themselves thrust into the spotlight of a televised show due to the profound challenges they faced in their intimate life. While Annie had a penchant for exploring the realms of kinkiness, indulging in leather, whips, and even daring sex parties, Mau sought a simpler, more traditional sexual connection. This stark disparity in their desires created significant friction within their relationship.
However, beneath the surface of their sexual discord lay deeper issues stemming from Annie’s troubled past. As Mau began to grapple with the complexities of her experiences, he gained a profound understanding of the driving forces behind his wife’s insatiable sexual appetite. Through the guidance of Dr. Orna, he embarked on a journey of self-discovery, unraveling the intricate workings of his own mind. Over time, these two individuals managed to forge a path toward reconciliation and rediscovered the deep love that bound them together.

The intentions that once aligned between them, particularly regarding the profound responsibility of nurturing a child, underwent a remarkable transformation. While Lauren found herself immersed in a profound self-exploration following her transition, Sam, on the other hand, harbored a steadfast belief that the opportune moment had arrived to embark on the journey of parenthood. In its inaugural season, Couples Therapy introduced four diverse couples from various backgrounds, each grappling with unique challenges. The cast included DeSean and Elaine, Lauren and Sam, Annie and Mau, and Alan and Evelyn, who all courageously shared their struggles with the camera and worked through them with their therapist. Throughout the season, they faced the challenging process of navigating their relationship issues and striving toward healing and growth.
Despite their love for each other, the queer and trans couple, Lauren and Sam, faced significant challenges in their relationship. Their differing views on when to have a child became a major point of contention, with Lauren wanting to focus on her own identity while Sam felt it was the right time to start a family. Despite seeking therapy to work through their issues, they were unable to find common ground and ultimately decided to end their relationship.
At first glance, Alan and Evelyn appeared to have little in common, leaving one to question what brought them together in the first place. Their differences led to difficulty in building trust, but despite this, they stayed together for six years. However, problems arose in the form of infidelity. Despite seeking counseling, they were unable to find the emotional stability to mend the damage and apologize to one another, ultimately leading to the breakdown of their marriage.
After being together for over a decade, DeSean and Elaine found themselves in couples therapy due to their differences in personality and emotional struggles. During therapy, Dr. Orna was frustrated with their lack of progress and inability to compromise on common issues. However, they eventually realized that they were too focused on their past instead of looking toward the future. Once they identified this problem, they were able to work through it and save their relationship, choosing to stay together.
Annie and Mau joined the show because they were struggling with their sexual compatibility. Annie had a desire for kinky sex, including leather, whips, and sex parties, while Mau preferred plain sex. However, as they delved into their past traumas, Mau came to understand the reasons behind Annie’s high sex drive. With Dr. Orna’s guidance, he was able to overcome his own inhibitions and become more open to exploring different aspects of their sexuality. Through therapy, they were able to strengthen their emotional connection, find common ground, and rediscover the love they had for each other. As a result, Annie and Mau managed to heal their relationship and are still together.
